Medical Records Request

Patients can request copies of their medical records by completing an Authorization for the Release of Medical records. There is a $1.00 fee for each page that is forwarded to the patient. There is no change for records forwarded to a physician. For your convenience, you can download this form by clicking on the link above or you may pick up one at our office. Medical records are copied on Tuesdays and mailed to the address provided.
