Be Prepared for Your Appointment

In order for us to best serve you, it is important to be prepared for your appointment.  Please review the information below and if you have any questions please contact our office.

Please bring these items to your appointment:

• Photo Identification
• Insurance Card(s): We provide insurance verification, but it’s a good idea for you to know ahead of time what is covered by your insurance company.
• Co-pay:  Your co-pay and any applicable deductible are due at the time of your visit.  We will verify your co-pay at the time of your appointment; however, if you would like to know what your co-pay is in advance, contact your insurance company and ask what your co-pay is for a specialist visit.  We accept these payment types: cash, check, visa & mastercard.
• Current MRI or X-rays: A current MRI or x-ray is one that has been taken within 6 months.  For your convenience, if a current x-ray is needed, we have an onsite x-ray technician.
• Patients Forms:  Please download patient forms and bring them with you to your appointment to reduce your wait time. *New Patients should download all the forms, returning patients with a new condition need to complete the Patient Problem Questionnaire and Medical History.
